School Based Mental Health Resources

Hazel Health

• This service is designed to support the well-being of our students, with the goal of fostering a healthy and positive school environment. Hazel will be providing telehealth services at school and at home.
• Free online counseling and therapy appointments
• Consent Required: No care will be provided without explicit guardian consent, except in cases where the student is age +18.
•  $0 Cost: Hazel visits are completely free to all students, regardless of their immigration status, insurance coverage, or financial situation.
• Licensed Therapists: Students can work 1-on-1 with a locally licensed therapist through weekly video sessions. These sessions can be done either at school or at home, depending on what works best for the student and their family. Services are available throughout the summer.
• Families can consent to and access these services at Hazel Health starting on Monday, June 2nd.

Guided Ascent

• Beginning on August 25, 2025, our experienced Board Certified Psychiatric Mental Health Nurse Practitioner, Tonya Puski, will begin visiting Perry elementary and middle/high school three times weekly throughout the school year.
• Tonya will provide supportive psychotherapy as well as medication management during these visits. We welcome parents/guardians to join the initial visit, virtually or in-person, and communication will continue throughout the 2025-2026 school year via email or phone as indicated.
• Appointments will take place within the school buildings and during school hours to provide minimal disruption to their learning experience.
• Guided Ascent will bill individual insurance plans for the appointments and offer affordable self-pay plans if necessary.
