Vaccination Requirements

Medical Requirements

Pre-school:
4 doses DTaP
3 doses Polio
3 doses Haemophilus Influenzae type B (HIB) - final dose on or after 12 months of age
4 doses Pneumococcal
1 dose MMR
1 dose of Varicella

Kindergarten:
5 doses of DTaP - one dose on or after 4 years of age
4 doses of Polio - one dose on or after 4 years of age
2 doses of MMR
3 doses of Hepatitis B
2 doses of Varicella

Other requirements for Kindergarten:
Dental screening:
The screening must occur no earlier than age 3 and no later than four months after enrollment. A licensed dentist, dental hygienist, physician, physician assistant, RN or ARNP can complete the screening. The screening must use the IDPH Certificate of Dental Screening.

Blood Lead screening:
All children are required to have at least one lead test. There is no need for parents to provide this health record to your child’s school. Schools will inform parents of children who need to be tested.

Vision Screening:
All children are required to have a vision screening no earlier than 1 year prior and no later than six months after enrollment. An ophthalmologist, pediatrician, physician or professional licensed for this test can perform the screening.

3rd Grade Requirement:
Vision Screening: All students must have a vision screening no earlier than 1 year prior to enrollment and no more than 6 months after. An ophthalmologist, pediatrician, physician or professional licensed for this test can perform the screening.

Middle and High School
Before the child starts school all 9th graders need a dental screening , 7th graders need T-dap and MCV4P vaccine, 12th graders need MCV4P.

Reminder to all parents of 12-18 year old that the covid vaccine is available to them.  They just need to seek their Dr. care or anywhere who gives  the pfizer vaccine.
